Harmonic Power Doppler US versus Conventional Power Doppler US with Use of Contrast Agent : Comparison in the Assessment of Vascularity in HCC

Abstract
Purpose : We compare the usefulness of second harmonic PD US with conventional PD
US in detection and characterization of tumor vascularity in nodular or massive hepatocellular carcinoma.

Materials and Methods : Seventeen patients with nodular with nodular or massive HCC(2.6-13.2cm; mean, 4.8cm) were prospectively examined with both conventional PD US and second harmonic PD US. Serial dynamic scans for 5 min were obtained before and after intravenous injection of the contrast agent(Levovist 300 mg/mL) with both techniques.

Results : The most common pattern of contrast enhancement on both techniques was linear and dot-like Doppler signals in the periphery and center of the tumor(76%). The amount of intratumoral signals was similar on both techniques at 30-90sec after injection of contrast agent; however, after 90 sec conventional PD US depicted more intratumoral signals than second harmonic PD US in most cases. Second harmonic PD US was superior to conventional PD US in terms of PD artifact such as motion-related artifact or blooming artifact in all cases.

Conclusion : As compared with contrast-enhanced conventional PD US, second harmonic PD US has the advantage of less PD artifact ; however, needs higher concentration of contrast agent to depict comparable intratumoral flow signals to conventional PD US. The duration of contrast enhancement of the tumor on second harmonic PD US is shorter than that on conventional PD US.
